Governing rule: no meeting without a decision or an output. Standups feed the boards rather than report on them, weekly reporting rolls into one integrated view, and governance itself is retrospected each quarter.

Items escalate upward through each forum; decisions flow back down. Each row shows where a decision is owned and where it goes if it cannot be resolved there.

Escalation SLA: a blocker raised in a daily standup reaches the PMO within 24 hours and the sponsor check-in within 5 business days; if still unresolved it goes to SteerCo / RAID Review.
